This reading conference form will help you stay organized and jot down meaningful notes about your students’ reading habits during reading conferences and small group meetings.

During conferences, it’s important to start by sharing a positive observation that you noticed about the students’ reading, followed by a quick lesson that can teach them something to improve their reading. Therefore, included on this form is space for a compliment, teaching point, and any other notes that you may want to jot down.

You can go paperless and use the Google Forms version of this form or print the PDFs and keep them on clipboards or in a notebook.
